Community Outreach Services
Our Community Outreach Team supports older people whose physical condition or functional mental health has deteriorated following a fall or bereavement or major life changing event resulting in isolation. We help them cope with their condition at home and reduce the risk of them being admitted, often unnecessarily, to hospital or prematurely entering residential care.
Our Hospital Discharge Service can offer a life-line to people who need that extra bit of support and encouragement following a hospital stay. For many older people, bereavement or illness, even a fall, can result in depression, a loss of confidence and independence.
Our Support@Home Service provides help, encouragement and support to people who may be struggling to live independently and may be at risk of homelessness. They may be struggling with the effects of substance misuse or the challenges of a new situation or diagnosis, such as dementia. Our aim is to help older people remain living in their own home for as long as they wish and is possible, by providing one to one support to continue doing the things they enjoy and that keep them healthy and independent.Each year we provide over thousands of hours of support to older people, helping them maintain or regain their independence.

The services described above are funded by our local public sector partners and as such they are free at the point of delivery.
